The most proficient answer is the bible. The fear of God will lead to following his word. His word has a lot of strict rules to discipline your mind, body, and spirit, and a guarantee into Heaven if you follow his way through Jesus Christ.
Other ways are:
Military – Armed forces like the Navy, Marines, and Army will guide you into a certain career that will direct your footsteps and get you ready for the trials and ways of life. You’ll make friends along the way as well.
Focus – Know what you want, know what you are trying to do and create a mental visual laser beam on what you want to accomplish. Block everything else out as you think only about what you want. View everything else as a side distraction.
Hypnosis – go to a professional hypnosis. (make sure you bring along a friend to make sure the hypnosis does what they’re supposed to do correctly.)
(Personal) Hypnosis – Go to Mind Master.com and purchase their program. Use that and also tell yourself the type of discipline you’re trying to achieve over and over again, till it fills your mind with nothing but that. Create and visualize mental images and illusions of walls that block out interferences while you visualize your self in a room with nothing but you and your goal.
Restrain Desire – Stay away from your most extreme desires. Ex: If you like food: – Fast for 3 days (meaning stay away from eating for 3 days). If you like to play video games stay away for a month. If you love music refrain from it or turn it off right when you’re in the middle of it or when your at your peak of listening to it. Stay away from your love ones for a while if you’re addicted to them.
Meditate – Take a form of martial arts to help restrain and control your inner energy. Kung Fu, Tia Chi, Pekiti Tersia Kali, Hopkido, Wushu, Shaolin, Yoga, etc.
Read – Reading helps the clarity of your mind.
Video Games – People may not believe this but, video games actually help with eye and hand coordination working similar in the ways leading to discipline. It helps with reaction in the same way of martial arts and pin point accuracy depending on what type of games you play. Our military are starting to use digital simulations to train our soldiers before they even enter the field of war. Goes to show you just how far games have advanced and go these days. Think about it, Dead or Alive, Shenmue, and Tekken have real time reaction counters, blocks, and real martial art styles in them. They teach more than mindless button mashing when used correctly and timed correctly.

Recall all consequences that relate to the past self indiscipline.
How can discipline changes the above situation?
You will gain by not repeating the past.
Discipline increases your self respect.
